This form records your pet and owner information into our management system so we can effectively and efficiently provide the service you deserve.

Surgical Information Packet

This form helps us understand the history and current state of your pet's health, as well as any questions, concerns, or goals you have regarding wellness.

What To Expect At Your First Visit

Current medications or supplements – Bring the actual bottles if possible

Don’t have complete records? That’s okay! Dr. Khabra can work with whatever information you have. Just share everything you know about your pet’s health history.

Your First Appointment

What We’ll Do:

Dr. D. Khabra will perform a comprehensive head-to-tail physical examination including:

  • Eyes, ears, and mouth evaluation
  • Heart and lung assessment
  • Abdominal palpation
  • Skin and coat inspection
  • Joint and mobility check
  • Weight and body condition assessment


What to Bring:

Completed new patient forms (or arrive 10 minutes early to complete them)

Medical history – Any records from previous veterinarians, including:

  • Vaccination records
  • Previous diagnoses or treatments
  • Current medications
  • Surgical history

Health journal – If you keep notes about your pet’s health, including:

  • Behavioral changes
  • Eating/drinking habits
  • Any concerns or symptoms

What Happens Next

After your examination, Dr. Khabra will:

  • Discuss findings with you
  • Answer all your questions
  • Recommend any needed diagnostics or treatments
  • Create a personalized care plan for your pet
  • Provide vaccination schedules if needed
  • Discuss preventive care options

In some cases, Dr. Khabra may refer you to a specialist who has advanced expertise in areas such as internal medicine or complex surgery. We’ll coordinate all referrals and ensure seamless care.

Annual Wellness Exams

We recommend bringing your pet in for a check-up at least once a year. Annual exams include:

  • Complete physical examination
  • Vaccination updates as needed
  • Parasite prevention recommendations
  • Dental evaluation (teeth and gum cleaning if needed)
  • Health screening discussions
  • Early detection of potential problems

For senior pets (10+ years), we recommend wellness exams every 6 months as health can change quickly in aging pets.

Payment Options

Payment Policy

Payment is expected when services are rendered. To focus on patient care, exceptional customer service, and keeping costs low, we do not bill for services.

Pet Insurance

Pet insurance can significantly reduce the cost of veterinary care by reimbursing 70-90% of covered expenses.

We provide:

Payment at time of service is required. You then submit claims to your insurance company for reimbursement.

Plan for 30-45 minutes for a comprehensive new patient exam. This allows time for thorough examination and discussion.

Absolutely! We encourage you to be present during the examination. Dr. Khabra will explain findings and answer questions as he examines your pet.

Yes, for urgent concerns we strive to offer same-day appointments. Call us as soon as possible.

Dr. Khabra will refer you to trusted specialists when advanced expertise is needed. We coordinate all referrals to ensure seamless care.

We require payment at time of service, but we provide all documentation needed for you to submit insurance claims for reimbursement.
